Across the country—and especially in New York—there has been a steady and deliberate shift in how care is delivered. The focus has moved away from institutional settings and toward more community-anchored, home-based, and person-centered models. This transition reflects broader national trends, driven by both policy and population needs, that prioritize integrated care, value over volume, and partnerships that meet people where they are.
In this session, we’ll explore how home and community-based services are positioned to play a central role across the care continuum—from prevention and chronic care management to post-acute and end-of-life care. We’ll also look at how payment innovation, 1115 waivers, social care integration, and models like ACOs and PACE are creating new opportunities and challenges for home care providers. Drawing on recent developments at both the federal level and in New York State, this discussion will offer a strategic perspective on how the landscape is evolving—and how home care can continue to lead.
